Adopter understands that he or she is responsible for the care of the animal until he or she has delivered it into the proper hands, and will do everything within his or her power to safely return the animal. He or she agrees that in the event he or she finds a suitable alternative home for the animal, he or she will contact Safe Haven with the name, address and phone number of the proposed new adopter. Agree Adoption Donation * Adopter agrees to pay a non-refundable adoption donation of $99 for cats over 1 year of age or $109 for cats under 1 year of age to help defray Safe Haven's expenses for food, sheltering and veterinary care. Additional donations are welcome. Agree Veterinary Care * Adopter agrees to provide the animal with the necessary vaccinations as advised by his or her veterinarian. Adopter agrees to obtain immediate veterinary care should the animal become sick or injured, follow the veterinarian’s treatment plan and take full financial responsibility for such veterinary expenses. Agree Care of the Animal * Adopter understands that the animal he or she is adopting is an indoor only pet, and agrees to allow it full access to his or her home (only fully enclosed patios). The adopter agrees that there will never be access to a functioning doggy door. Adopter agrees to provide the animal with fresh water, wholesome food, adequate exercise and affection. Agree Declawing * Adopter agrees NOT to declaw the cat, understanding that declawing is the painful amputation of the first knuckle, which renders the animal defenseless and inflicts unnecessary pain. Agree No Representations * Adopter understands that Safe Haven does not guarantee the health, temperament, or training of the above described animal, and Adopter hereby holds Safe Haven, its agents and employees, harmless from any claim, cause of action or liability for any injury or damage to persons or property in connection with the adoption or ownership of the animal. If a health problem with the animal develops, Adopter agrees to notify Safe Haven to discuss the matter. Agree Change of Address * In the event that Adopter moves, he or she agrees to contact Safe Haven with change of address information, so that all records can be updated. Agree Reclaimation * Safe Haven for Animals makes a lifetime commitment to any animal that comes into our care. Realizing that on the rare occasion, Safe Haven may have to reclaim the adopted animal, there is a procedure that would be followed: Safe Haven will notify Adopter by certified mail, return receipt requested, of the reclamation and ask that the animal be returned. The notice shall state clearly the circumstances under which Safe Haven is acting and state a date when Adopter may meet with agents of Safe Haven to discuss the situation. After seven days from the posting of this notice, if Adopter has not notified Safe Haven, or has not accepted the notice, legal action will be taken by Safe Haven to retrieve the animal. If the animal is in such an extreme circumstance as to endanger its life, Safe Haven may call animal control in Adopter's locality to recover the animal on behalf of Safe Haven. In the event either of the foregoing situations occurs, or if for any reason the animal is returned to Safe Haven, Adopter may request a meeting to discuss the matter within ten days after the animal is reclaimed. Any decision of Safe Haven is final as to whether the animal shall be returned to the Adopter. Agree Home Inspection * Adopter agrees to an in home facility inspection prior to the completion of any adoption. In addition, adopter agrees to periodic phone calls or in person wellness checks. Twenty four hours notice will be given prior to an in home inspection. Agree THIS AGREEMENT is the entire agreement between Safe Haven and Adopter, and any prior understandings between the parties with respect to the subject matter of this Agreement. No modification of this Agreement will be valid unless in writing and duly signed by Safe Haven and the Adopter. This Agreement is binding upon the heirs, assigns, successors, personal representatives and executors of both parties. Electronic Signature Consent * By electronically signing any document related to Safe Haven for Animals, you acknowledge and agree that your electronic signature carries the same legal effect as a handwritten signature. You consent to conduct transactions electronically and understand that all records, agreements, and communications in electronic form are legally binding. Safe Haven for Animals reserves the right to request physical signatures if necessary.
